I cannot remember when I purchased my fans and I failed to fill out the PROOF OF PURCHASE form and mail it in,but I have the blank card for them. My model #'s are (10177 with remote control) and model #10176. I just wanted to say that these fans are still working and are in great shape and I have not had any problems with them in all these years except for misplacing my remote control. WHAT A CREAT PRODUCT !!!
