FastField is the most flexible, user friendly app there is for making any kind of form you can imagine. On top of that you don't have to be an ace programer to use it.

Pointless update Feedback
I have repeatedly provided feedback requesting a return to the simplicity of the previous version. Labelling of photos got twice is labour-intensive as a result of the last update. I won’t be providing feedback again
